Mid Career (Above 6 yrs exp)

We are seeking a Professional Nurse to provide comprehensive nursing care to patients with life-limiting illnesses. As a key member of the team, you will collaborate with interdisciplinary teams, provide emotional support to patients and families, assess referrals, manage admissions, and develop staff off-duty rosters.

• Deliver comprehensive nursing care to patients with life-limiting illnesses
• Collaborate with an interdisciplinary team to develop and implement plans
• Provide emotional support to patients and their families
• Assess and coordinate new referrals
• Manage admissions and discharges
• Plan and manage staff off-duty rosters
• File audits
• Attend Monthly meetings
• Cross functioning between the Inpatient Care Facility and Home Nursing program
• Willingness to work overtime, day and night duty, weekends and ph to meet operational requirements
