- Summary : The Clinical Facilitator provides clinical expertise to patients, staff and providers in all medical specialties across the health system through phone consultations, virtual visits, and on-site support. The position works closely with interdisciplinary teams including nursing, social work, physicians, and other supportive care services to improve quality of life for both inpatients and outpatients at all stages of illness. This position requires strong communication skills, excellent judgment, empathy, and critical thinking skills. Responsibilities :
- Provide high quality patient centered telephone triage and consultation for acute, chronic, and palliative conditions
- Participate as an active member of the interdisciplinary team providing inpatient consults, virtual visits, and outpatient support
- Coordinate with social workers, home health, hospice, and other community resources to ensure appropriate discharge planning
- Collaborate with colleagues and leadership to identify opportunities to enhance the quality of care provided by the service
- Support program development and evaluation efforts related to improving symptom management and advance care planning
- Teach students, residents, fellows, and other learners in the art and science of palliative medicine
- Perform duties within scope of license as a Physician Assistant or Nurse Practitioner Qualifications :
- 12-month trained ICU course
- Current licensure as a Physician Assistant (PA) or Nurse Practitioner (NP)
- Eligibility for DEA registration
- Excellent verbal and written communication skills
- Demonstrated ability to provide compassionate, culturally sensitive, patient centered care
- Experience in a hospital setting preferred
- Board certification in Hospice & Palliative Medicine, Internal Medicine, Family Medicine, Geriatrics, or Pediatrics
- Demonstrated commitment to working with diverse populations
- Proven ability to work independently and collaboratively
- Ability to maintain confidentiality and professional boundaries
