About the Role

Reporting to the Therapy Manager, our Social Worker is an essential member of the interdisciplinary rehabilitation team. You will :

• Perform comprehensive psychosocial assessments and craft individualised care plans.
• Provide counselling, crisis intervention and ongoing emotional support to patients and families.
• Facilitate family meetings, discharge planning and smooth community reintegration.
• Advocate for resources, benefits and funding to optimise long-term outcomes.
• Collaborate daily with the interdisciplinary team to deliver seamless evidence-based care.
• Maintain accurate, confidential documentation compliant with SACSSP standards and contribute to quality-improvement initiatives.
