Case Manager - Youth Development

Key Responsibilities

• Develop individualized case plans for youth based on their specific needs and goals.
• Provide ongoing support, mentoring, and guidance to youth navigating educational, personal, or social hurdles.
• Facilitate access to educational programs, vocational training, and employment opportunities.
• Conduct regular follow-ups to monitor progress and adjust support strategies as needed.
• Collaborate with schools, community centers, and other youth-serving organizations.
• Maintain accurate and confidential client records and prepare progress reports.

Requirements

• Bachelor's degree in Social Sciences, Psychology, or a related field.
• At least 2 years of experience working with youth in a social work or case management capacity.
• Proven ability to build rapport and establish trust with young people from diverse backgrounds.
• Strong understanding of adolescent development and common challenges faced by youth.
• Excellent organizational skills and attention to detail in record-keeping.
• Valid driver's license and willingness to travel within the Tembisa area.
