About the Role
Our client is looking for a skilled and motivated Case Manager to join their social services team in Port Elizabeth . This position requires a proactive individual who can effectively assess client needs, develop comprehensive service plans, and coordinate the delivery of essential support services. You will work with a diverse client base, helping them overcome challenges related to housing, employment, health, and social integration. Key Responsibilities
- Conduct comprehensive assessments to identify client needs and eligibility for services.
- Develop individualized case plans in collaboration with clients, setting clear goals and objectives.
- Coordinate and link clients with appropriate community resources and support services.
- Monitor client progress regularly and adjust service plans as needed.
- Maintain detailed case notes, client records, and prepare timely progress reports.
- Advocate on behalf of clients to ensure they receive necessary support and resources.
Requirements
- Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
- Minimum of 2 years of experience in case management or a related social service role.
- Knowledge of social welfare programs, community resources, and referral networks in Port Elizabeth .
- Excellent assessment, planning, and problem-solving skills.
- Strong organizational and time management abilities.
- Effective communication and interpersonal skills for building client relationships.
