About the Role
Our client is looking for a dedicated Support Coordinator to join their remote-first Disability Services team, based out of Stellenbosch . This crucial role involves assisting individuals with disabilities to access and coordinate the services they need to live fulfilling and independent lives. You will manage a portfolio of clients, develop personalized support plans, liaise with service providers, and ensure clients receive optimal care and support. Key Responsibilities
- Coordinate support services for individuals with disabilities, aligning with their goals and preferences.
- Develop, implement, and review personalized support plans in collaboration with clients and their families.

- Provide advocacy and support to clients navigating the disability support system.
- Maintain accurate and up-to-date client records and case notes through the online portal .
- Conduct regular remote check-ins and assessments to monitor client well-being and satisfaction.
Requirements
- Relevant qualification in Social Work, Occupational Therapy, Disability Studies, or a related field.
- Minimum of 3 years' experience in disability services or support coordination.
- In-depth knowledge of the disability sector, available resources, and relevant legislation.
- Excellent organizational and planning skills , with meticulous attention to detail.
- Strong interpersonal and communication skills, with proven ability to build rapport remotely.
- Proficiency with digital tools for communication and case management.
