Key Responsibilities

Requirements

• High school diploma or equivalent; certification as a Community Health Worker is a plus.
• Previous experience in a community health, social work, or healthcare support role.

• Excellent communication, interpersonal, and cultural competency skills.
• Proficiency with basic computer skills, mobile devices, and telehealth platforms for remote interactions.
