About the Role

Our client is seeking a proactive and community-oriented Community Health Navigator to join their team in East London . This hybrid role involves connecting individuals and families with essential health and social services. You will utilize a combination of remote outreach and in-person engagement to help clients overcome barriers to accessing care, understand their health options, and navigate the complex healthcare system. This role is crucial for improving health outcomes and promoting wellness within underserved communities. You will act as a trusted liaison, ensuring that community members receive the support they need to lead healthier lives.

Key Responsibilities

• Assist clients in identifying their health and social service needs.
• Guide clients through the process of accessing healthcare providers, specialists, and community resources.
• Provide information on health education, preventive care, and available health programs.
• Conduct follow-up with clients to ensure they are receiving necessary services and address any challenges.
• Build and maintain relationships with local healthcare providers and community organizations.
• Document client interactions and progress using the organization's case management system.

Requirements

• Relevant qualification in Community Health, Public Health, Social Work, or a related field.
• Previous experience working in community outreach, health navigation, or social services.
• Strong understanding of the local healthcare system and community resources.
• Excellent communication, interpersonal, and motivational interviewing skills.
• Ability to work independently and collaboratively within a hybrid model.
• Proficiency in using technology for remote communication and data management.
