About the Role
Our client is seeking a dedicated Rehabilitation Coordinator to join their team in Mahikeng . This role is crucial for overseeing programs designed to help individuals recover from physical, mental, or substance abuse challenges, enabling them to regain independence and improve their quality of life. You will work with clients to develop personalized rehabilitation plans, coordinate services, and support them throughout their recovery journey. This is a vital position serving the community of the North West province, promoting health, recovery, and social reintegration.
Key Responsibilities
- Assess clients' needs and develop comprehensive, individualized rehabilitation plans.
- Coordinate services with healthcare providers, therapists, social workers, and other support professionals.
- Monitor client progress, adjust treatment plans as necessary, and provide ongoing support.
- Educate clients and their families about the rehabilitation process and available resources.
- Liaise with external agencies and community organizations to facilitate client reintegration.
- Maintain detailed records of client assessments, treatment plans, and progress notes.
Requirements
- Bachelor's degree in Social Work, Psychology, Occupational Therapy, or a related health/social science field.
- Experience in case management or coordination within a rehabilitation setting.
- Strong understanding of various rehabilitation methodologies and recovery principles.
- Excellent organizational, communication, and interpersonal skills.
- Ability to work effectively with diverse client populations and multidisciplinary teams.
- Knowledge of support services available in the Mahikeng region.
