About the Role

Our client is seeking a compassionate and organized Disability Support Coordinator to join their team in Bloemfontein . This role is central to empowering individuals with disabilities to live fulfilling and independent lives. You will work closely with clients, their families, and various service providers to develop and implement personalized support plans. Key Responsibilities

• Conduct thorough needs assessments for individuals with disabilities, identifying support requirements and goals.
• Develop, implement, and regularly review individualized support plans in collaboration with clients and their support networks.
• Coordinate access to essential services, including healthcare, therapy, education, employment support, and community integration activities.
• Act as a primary point of contact and advocate for clients, ensuring their rights and needs are met.
• Liaise effectively with service providers, government agencies, and families to ensure seamless service delivery.
• Maintain accurate client records, case notes, and progress reports in compliance with organizational standards.

Requirements

• A relevant qualification in Social Work, Psychology, Occupational Therapy, or a related field.
• A minimum of 3 years of experience in disability support services or case management.
• In-depth knowledge of disability types, relevant legislation, and available support services.
• Excellent communication, interpersonal, and problem-solving skills.
• Strong organizational abilities and experience in coordinating multiple services.
