Community Health Worker

About the Role

Our client is seeking a compassionate and dedicated Community Health Worker to serve the residents of Mossel Bay, Western Cape . This essential role involves providing health education, basic health screenings, and support services directly within the community, bridging the gap between healthcare facilities and individuals. You will act as a vital link, promoting healthy lifestyles, facilitating access to healthcare services, and advocating for the health needs of the community. This position requires an individual with a strong commitment to public health and a desire to make a significant impact on the well-being of individuals and families in the Mossel Bay area.

Key Responsibilities

• Conduct health education sessions on topics such as hygiene, nutrition, disease prevention, and maternal/child health.
• Perform basic health screenings, including vital signs monitoring and glucose testing, as trained.
• Assist community members in accessing primary healthcare services, clinics, and hospitals.
• Provide support and follow-up care for patients with chronic conditions.
• Conduct home visits to assess living conditions and provide health advice.
• Collect community health data and report findings to the local health department or clinic.

Requirements

• Completion of a recognized Community Health Worker training program or equivalent experience.
• Understanding of local health issues and challenges faced by the community in Mossel Bay .
• Excellent communication and interpersonal skills, with the ability to build trust within diverse populations.
• Proficiency in at least one local language in addition to English.
• Ability to work independently and travel within the community.
• A commitment to promoting health equity and community well-being.
