About the Role
Our client is seeking a highly organized and compassionate Support Coordinator to join their community services team in Klerksdorp , operating on a hybrid model. You will play a pivotal role in connecting individuals with disabilities or complex needs to essential services and supports within the community. This involves assessment, planning, and coordinating care, ensuring clients receive tailored assistance to achieve their personal goals and enhance their quality of life. Your work will bridge the gap between clients and the services they need, requiring strong communication and problem-solving skills in both remote and face-to-face interactions.
Key Responsibilities
- Conduct thorough needs assessments for clients to identify their support requirements and goals.
- Develop personalized support plans in collaboration with clients, their families, and relevant stakeholders.
- Coordinate and facilitate access to a range of community services, including healthcare, housing, and social programs.
- Monitor the effectiveness of support plans and make adjustments as needed through regular reviews.
- Liaise with service providers, government agencies, and advocacy groups to ensure seamless support delivery.
- Maintain comprehensive case notes and documentation, adhering to confidentiality protocols.
Requirements
- Relevant qualification in Social Work, Community Services, Disability Studies, or a related field.
- Proven experience in disability support , case management, or community services coordination.
- Strong understanding of the disability sector and available support services.
- Excellent assessment, planning, and problem-solving skills.
- Proficiency in using client management systems and standard office software.
- Ability to communicate effectively with diverse individuals and stakeholders.
