Social Support Facilitator

Our client is looking for a proactive and compassionate Social Support Facilitator to provide crucial assistance to individuals and families in the Richards Bay area. This role involves assessing needs, coordinating access to essential services, and offering practical support to enhance client well-being and community integration. You will serve as a key point of contact, connecting clients with resources related to housing, health, education, and employment. The successful candidate will have a strong commitment to community service and a deep understanding of the social landscape in KwaZulu-Natal .

Key Responsibilities

• Conduct thorough needs assessments for individuals and families seeking support services.
• Facilitate access to community resources, including social grants, healthcare, and educational opportunities.
• Provide practical assistance and guidance to clients navigating complex support systems.
• Maintain accurate and confidential client case files and progress reports.
• Liaise with relevant government departments and non-profit organizations to advocate for clients.
• Organize and lead community engagement activities aimed at addressing specific social needs.

Requirements

• Relevant qualification in Social Work, Community Development, or a related field.
• At least 2 years of experience in a social support or case management role.
• Knowledge of local social services and community structures in Richards Bay .
• Strong empathy, active listening, and communication skills.
• Ability to manage a diverse caseload and prioritize tasks effectively.
