About the Role

Our client is looking for a dedicated Community Health Navigator to join their team in Randburg, operating with a flexible hybrid work model. In this vital role, you will act as a bridge between individuals and the healthcare system, helping them access necessary medical services, understand their health conditions, and navigate complex healthcare pathways. You will combine remote support with essential in-person interactions to ensure clients receive comprehensive and accessible care. Your efforts will directly contribute to improved health outcomes and greater health equity within the community.

Key Responsibilities

• Assist clients in understanding their health needs and navigating the healthcare system.
• Facilitate appointments with healthcare providers, specialists, and community health resources.
• Provide education on preventive health measures and chronic disease management.
• Help clients overcome barriers to accessing care, such as transportation, language, or financial concerns.
• Maintain confidential client records and track progress towards health goals.

Requirements

• Bachelor's degree in Health Sciences, Public Health, Social Work, or a related field.
• Experience in healthcare, public health, or community services.
• Strong knowledge of the local healthcare landscape and available community resources.
• Excellent communication, empathy, and problem-solving skills.
• Proficiency in using electronic health records and virtual communication tools.
• Ability to work independently and manage a caseload effectively in a hybrid environment.
