Disability Support Coordinator

About the Role

Our client is looking for a proactive Disability Support Coordinator to join their team in Nelspruit . This hybrid role focuses on empowering individuals with disabilities to live more independent and fulfilling lives. You will work closely with clients and their families to understand their needs, develop personalized support plans, and connect them with essential services and resources within the community. This role requires a compassionate individual who is adept at navigating various support systems and advocating for the rights and needs of people with disabilities in the Nelspruit area.

Key Responsibilities

• Conduct comprehensive assessments of clients' support needs, abilities, and goals.
• Develop individualized support plans in collaboration with clients, families, and caregivers.
• Coordinate access to a range of services, including healthcare, housing, education, and employment.
• Provide ongoing monitoring and support to ensure the effectiveness of the support plan.
• Advocate for clients' rights and needs within various service systems.
• Maintain accurate and confidential client records and case notes.

Requirements

• Diploma or Bachelor's degree in Social Work, Psychology, Occupational Therapy, or a related field.
• Minimum of 2 years of experience in disability support or case management.
• Strong understanding of disability types, support services, and relevant legislation.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work independently and collaboratively in a hybrid work environment.
• Proficiency in using client management systems and digital communication tools.
