Social Worker - Hybrid Role

About the Role

Our client is seeking a dedicated Social Worker to join their dynamic team, offering a blend of remote flexibility and essential on-site engagement in George . In this role, you will assess client needs, develop intervention plans, and provide crucial support services to individuals and families facing various challenges. You will leverage technology for remote case management and communication while also engaging in face-to-face interactions when necessary to build stronger client relationships and provide more comprehensive care. Key Responsibilities

• Conduct comprehensive assessments of client needs, including psychosocial, environmental, and financial factors.
• Develop and implement individualized service plans in collaboration with clients and their families.

• Liaise with community agencies, healthcare providers, and other stakeholders to coordinate care.
• Maintain accurate and timely case records, ensuring compliance with all relevant regulations.

Requirements

• Bachelor's degree in Social Work or a related field.
• Professional registration with a relevant South African council (e.g., SACSSP).
• Minimum of 3 years of experience in social work practice.
• Proficiency in case management software and telehealth platforms . Excellent interpersonal, communication, and crisis intervention skills.
• Ability to work effectively both independently and as part of a multidisciplinary team.
