About the Role

Our client is looking for a highly organized and dedicated Rehabilitation Coordinator to manage and oversee rehabilitation programs for individuals recovering from injury or illness, serving the East London region entirely remotely. This role is critical in ensuring clients receive comprehensive and effective support throughout their recovery journey. You will coordinate services, liaise with healthcare providers, and connect clients with necessary resources, all through a virtual platform. Key Responsibilities

• Develop and coordinate individualized rehabilitation plans for clients based on their specific needs and goals.
• Liaise with healthcare professionals, therapists, and external agencies to ensure seamless service delivery.
• Monitor client progress throughout their rehabilitation program, making adjustments as necessary.
• Provide ongoing support, guidance, and education to clients and their families regarding recovery.
• Connect clients with appropriate community resources, support groups, and assistive devices.
• Maintain detailed and accurate client records, documenting interventions and outcomes.

Requirements

• Professional qualification in a relevant field such as Occupational Therapy, Physical Therapy, Social Work, or a related health science.
• Minimum of 4 years of experience in rehabilitation coordination or case management.
• Strong understanding of various rehabilitation processes and client needs.
• Excellent organizational, communication, and problem-solving skills.
• Proficiency in using telehealth platforms, electronic health records, and remote communication tools.
• Ability to manage a caseload independently and maintain a high level of client focus.
