Key Responsibilities
- Assist community members in understanding health conditions, treatment options, and preventative care strategies.
- Guide clients through the process of accessing healthcare services, including appointments and insurance enrollment.
- Provide information on available health resources, support groups, and community health programs.
- Conduct virtual needs assessments to identify barriers to healthcare access and develop tailored action plans.
- Maintain accurate and confidential client records and document all interactions and referrals.
- Act as a liaison between clients and healthcare providers to ensure effective communication and follow-up.
Requirements
- Bachelor's degree in Public Health, Health Sciences, Social Work, or a related field.
- Minimum of 2 years of experience in health navigation, community health work, or a similar role.
- Strong knowledge of the healthcare system and available community health resources.
- Excellent virtual communication and interpersonal skills, with empathy and cultural competence.
- Proven ability to work autonomously and manage time effectively in a remote environment.
- Proficiency with telehealth platforms and electronic health record systems.
