Key Responsibilities
- Assess patient needs and develop comprehensive, individualized care plans in collaboration with healthcare teams.

- Liaise with insurance providers to obtain authorizations for services and manage coverage issues.

Requirements
- Current and unrestricted Registered Nurse license.
- Minimum of 5 years of experience in clinical nursing, with at least 2 years in case management or care coordination.

- Excellent assessment, planning, and problem-solving skills.
- Proficient in using electronic health record (EHR) systems and telehealth platforms.
