Key Responsibilities
- Case management for chronic conditions : HIV, TB, Diabetes, Hypertension, COPD, Asthma, Mental Health, CKD, Cardiac .
- Patient onboarding for HIV / TB and chronic programmes.
- Approve HIV / TB medications per NDoH and clinical guidelines.
- Conduct adherence counselling (telephonic / face-to-face).
- Develop and maintain care plans; support treatment compliance.
- Liaise with treating providers to ensure quality clinical outcomes.
- High-risk case management : discharge planning, continuity of care, home-based care coordination.
- Authorise home nursing, wound care, palliative care, stoma therapy.
- Oncology : patient registration, treatment plan authorisation (ICON guidelines), coordination of reviews.
- Maternity : programme registration, risk assessment, patient education, post-partum follow-up, NIPT authorisation.
Minimum Requirements
Nursing Degree or Diploma (essential).Registered Professional Nurse (SANC) .Matric / National Senior Certificate.2+ years clinical experience in a PRIVATE hospital environment (essential)HIV / TB management experience per NDoH (advantageous).Managed care / medical scheme experience (advantageous).Primary healthcare or wound / diabetic care exposure (advantageous).Computer literacy : MS Office.Core Competencies
