Case Manager : Disease Management

Key Responsibilities

• Case management for chronic conditions : HIV, TB, Diabetes, Hypertension, COPD, Asthma, Mental Health, CKD, Cardiac .
• Patient onboarding for HIV / TB and chronic programmes.
• Approve HIV / TB medications per NDoH and clinical guidelines.
• Conduct adherence counselling (telephonic / face-to-face).
• Develop and maintain care plans; support treatment compliance.
• Liaise with treating providers to ensure quality clinical outcomes.
• High-risk case management : discharge planning, continuity of care, home-based care coordination.
• Authorise home nursing, wound care, palliative care, stoma therapy.
• Oncology : patient registration, treatment plan authorisation (ICON guidelines), coordination of reviews.
• Maternity : programme registration, risk assessment, patient education, post-partum follow-up, NIPT authorisation.
