Job descriptionConduct comprehensive reviews of chronic medication regimens for accuracy, appropriateness, and compliance with client protocols. Identify potential drug interactions, duplications, and Ensure documentation and reporting meet regulatory and client standards. Provide evidence-based recommendations for therapy optimization in chronic disease Collaborate with healthcare providers to address clinical queries and improve treatment Stay updated on clinical guidelines, formulary changes, and best practices in chronic Maintain high standards of accuracy and consistency in medication Participate in audits and implement corrective actions where Communicate effectively with clients, prescribers, and internal teams to resolve issues and enhance service delivery. Offer training and support on chronic medicine protocols when required. Strong clinical knowledge in pharmacology and chronic disease Attention to detail and ability to interpret complex medical information. Excellent communication and problem-solving Proficiency in healthcare systems and medication review Excellent verbal and written communication skills Analytical, systematic and detail orientated Have a professional and positive attitude Interpersonal and customer-orientated service skills Ability to work independently and in a multidisciplinary project team Ability to handle pressure and difficult situations Ability to work under set deadlines – time management is critical Be responsible and able to maintain confidentiality Be able to make decisions Good analytical and problem-solving abilities Good communication and interpersonal skills Good organisational and administrative skills Ability to meet tight deadlines Sound knowledge of the Medical Schemes Good knowledge of various legislation concerning medical schemes, health insurance, PMBs, and other healthcare Sound knowledge of the pharmacy / pharmaceutical industry
Position Purpose : To ensure accurate and timely processing of chronic medicine reviews in alignment with client specifications, while providing expert clinical input to optimize medication management and patient outcomes. To assist with operational activities within the PHA Medicine Management space.
Experience : Minimum of 1 year experience with capturing medicines in a DUR / PHB environment with
- years previous working experience within a Managed Care or Pharmaceutical environment (Essential).
Qualifications : Bachelor of Pharmacy. Registration with SAPC.
