We are seeking a dedicated and motivated Clinical Pharmacist to lead, manage, coordinate, and monitor clinical pharmacy services while driving pharmaceutical improvement within the hospital. The successful candidate will be responsible for conducting daily ward rounds, reviewing patient medication profiles across the hospital division, and contributing to improved patient outcomes.
Key Responsibilities
Conduct daily ward rounds, perform comprehensive patient medication reviews, monitor responses, and make interventions to ensure optimal therapeutic outcomes.
Ensure medication safety and resolve medication-related challenges in a timely manner.
Elevate antimicrobial stewardship, including monitoring microorganisms and analysing stewardship data to identify inappropriate antimicrobial use.
Drive interventions to optimise drug utilisation and pharmaceutical care.
Collaborate professionally with doctors, microbiologists, nurses, infection prevention control teams, and other stakeholders.
Provide evidence-based drug information, medication therapy management advice, and formulary management recommendations.
Actively participate in interdisciplinary discussions on patient care and medication management.
Maintain detailed patient records, documenting interventions, recommendations, and outcomes.
Develop and implement drug administration protocols to proactively manage risk.
Report on medication adverse events and interventions.
Mentor and support pharmacists to build skills, capacity, and professional competence.
Prepare and present reports to the Clinical Risk Committee.
Engage in continuous education, training, and operational research initiatives to enhance clinical knowledge and practice.
Minimum Qualifications and Experience
B. Pharm Degree and a postgraduate qualification in Clinical Pharmacy.
Active registration with the South African Pharmacy Council.
Minimum of five (5) years’ post-internship pharmacist experience, including at least two (2) years in Clinical Pharmacy with exposure to Antibiotic Stewardship, Pharmacovigilance, and Rational Medicine Use.
Proficiency in Microsoft 365 applications (Excel, PowerPoint).
Excellent written and verbal communication skills with the ability to collaborate effectively across multidisciplinary teams.
Experience with the Blue Bird programme and SAP will be advantageous.
