Community Services and Linkage Manager
mothers2mothers (m2m) is an African Primary Health Care organisation that employs women living with HIV as community health workers across African nations. These “Mentor Mothers” deliver integrated health services at clinics and in communities to ensure everyone, everywhere has access to the services they need to be healthy and stay in care. Over the next five years, this proven, peer-led model is being scaled up to double down on ending HIV, as well as to tackle new health challenges and reach more people—all with the aim of building a fairer, healthier future. The employment of local women as community health workers means that we are delivering a model that prioritises healthcare for families who need it most, delivered by women who know them best. Since its creation in 1, m2m has created jobs for nearly , women living with HIV in sub-Saharan Africa and, together, they have reached over M people with critical health services and education. Key Performance Areas
- Strengthen bi-directional referral systems to improve both health / HIV and social service delivery for OVCA&Y
- Strengthen implementation of comprehensive, HIV-inclusive case management with a focus on improving paediatric HIV case identification
- Strengthen HIV prevention focus, linkage to HTS, care, and treatment; retention among both children and their caregivers; and violence prevention and response.
- Manage community profile and mapping referral systems of patients from the community to facility and vice versa
- Work closely with stakeholders to create demand for health services
- Communicate with operational managers in health facilities to ensure that the services provided can meet the demand created
- Assist facilities to develop systems to enhance the linkage into care or patients referred from community testing events
- Facilitate the tracing of defaulters in conjunction with community deployed staff
- Provide administrative support to ensure that clients receive the appropriate care at each community visit.
- Ensure that linkage support aligns to community activities
- Engage communities to improve the quality of care retention in care and adherence to treatment
Qualifications and Experience
A minimum of Bachelor’s degree in Social Work or Public Health with a focus on public health, social sciences, or related field.A minimum of 5 years of progressively responsible experience in a technical advisory role for large-scale family and community-based programs, with a focus on vulnerable children and youth preferred.A minimum of 3 years of experience serving in a technical advisory role for PEPFAR-funded activity, with a focus on improving the wellbeing of orphans and other vulnerable children, adolescents and youth (OVCA&Y) and HIV prevention among adolescent girls and young women (AGYW).Demonstrated experience in strengthening coordination and linkages between clinical and community stakeholders, health and social service workforces; and operationalizing integrated service delivery models.Demonstrated working knowledge of PEPFAR Technical Guidance for OVCA&Y and HIV prevention.Demonstrated project management experience.Excellent written, presentation and communication skills in English.Working knowledge of siSwati and other local languages in Mpumalanga (e.g. isiZulu, isiNdebele, Northern Sotho, Xitsonga).
